Diabetes Management Program

Benefitted EKU employees and their covered dependents who are diagnosed with diabetes are eligible to participate in the Diabetes Management Program.  This program is coordinated by Kentucky One Health. Participants completing required sessions with the Diabetes Educator receive their diabetic supplies at no cost. 

A Diabetes Educator is available most Monday's & Thursday's in Rowlette 114-walk-ins are welcome schedule permitting, OR to ensure the time that best fits your schedule, call 859-313-2393 or by email.

New Participants

New program participants must attend a 1 hour initial session. Schedule your appointment by calling 859-313-2393 or by email.

At the initial session, a Diabetes Educator will provide:

  • an individualized diabetes assessment, weigh in and A1C as needed
  • explain the Diabetes Self Management Education (DSME) required to receive benefits

Individual Follow Up Sessions

Individual follow up sessions will be planned by employee at their convenience at their initial session, by calling 859-313-2393 or by email.
Participants must attend one per quarter to continue to receive benefits. 

At the follow up session:

  • bring your log book and meter
  • write down and bring questions you have
  • take sheet given by diabetes educator to HR to continue to receive benefits 

Diabetes Self Management Education  (DSME)

All classes are one hour per week for eight (8) weeks. Participants must attend 6 of 8 sessions to receive benefits.

When: Mondays & Thursdays, 11:00am & 2:30pm

DSME class content sessions: 

Week 1: Define Diabetes/Pathophysiology / Behavior Goal importance

Week 2: Nutrition – Carb; portion control/ plate method/ hand method

Week 3: Blood Sugar Testing (SMBG goal)

Week 4: Carb Counting / Behavior Goal setting

Week 5: Carb Counting 

Week 6: Acute chronic complications/sick days

Week 7: Heart healthy nutrition

Week 8: Exercise / Behavior Goal setting

ALTERNATE CLASS SITE AND TIMES AT SAINT JOSEPH BEREA and SAINT JOSEPH EAST ARE AVAILABLE.  This is a 3 class series and can be a substitute for the above weekly DSME sessions.  Ask the diabetes educator if interested.
