Biometric Screenings
Agreeing to a biometric screening/annual physical includes: height, weight, blood pressure, waist girth, and a blood panel consisting of total cholesterol, HDL, LDL, triglycerides and fasting glucose. This is voluntary. Your health information is protected by HIPAA.
- You may schedule an appointment at one of the BluMine Health locations or with another care provider to complete your biometric screening.
- Be sure to take the biometric screening form with you to your appointment. The biometric screening form must be completed to earn the $100 incentive. If taken at BluMine, they will do this part for you.
Your annual preventative appointment to your primary care physician is covered by the EKU health plan with a $0 co-pay. Visits to BluMine Health Clinic are also free of cost to employees covered by the EKU health plan and their dependents (ages 2-26).
